Most dental offices require new patients to fill out a patient information form, medical history questionnaire, and other relevant paperwork. Our office is no different. It is important to fill them out thoroughly and accurately, providing details about your medical history, current medications, allergies, and any dental concerns or past dental treatments.

To ensure a smooth and hassle-free experience during your visit, please have all pertinent information with you so we can confirm your identity, review your health history, and provide you with quality dental care.

This information includes but is not limited to:

  • a current government-issued form of photo identification;
  • your health information – including your medical and dental history;
  • a complete list of all your current medications – including prescription medications, over-the-counter drugs, and/or natural supplements – please include the full name, dosage, and reason for taking the medication.
  • a valid form of payment – we accept cash and most major debit and credit cards;
  • if you have dental benefits – any dental insurance information;
  • and any other information required for us to provide you with quality dental care.

Identification:  For your first visit with us, please bring a valid form of identification to confirm your identity. This can be a government-issued photo ID, such as a Driver’s License, Ontario Photo Card, Permanent Residence Card, or Passport. Additionally, you may bring your health card along.

Health Information:  We kindly request you to bring all your relevant personal information, including a detailed medical history, to your first dental visit. This information is crucial for our dental team to provide you with safe and effective care, as it helps us understand any existing medical conditions, medications, or allergies that may impact your treatment or require special considerations. Your health history enables us to tailor our approach specifically to your needs and ensure the best possible dental experience for you.

Medication Information: Please make sure to bring a detailed list of all the medications you are currently taking, including the names and amounts of each medicine, as well as why you’re taking them. This includes any prescribed medications, over-the-counter drugs, and any vitamins or supplements you may be using. If you have a preferred pharmacy, they can usually give you a complete list of your medications. Special Considerations:  If you have any special considerations or require pre-medication prior to your appointment, please inform us in advance. We want to ensure your comfort and make any necessary arrangements for your specific needs.

Dental Records:  If you’ve previously visited another dental office, it’s helpful to request your dental records to be transferred to our office. This will provide us with valuable insights about your oral health history, any ongoing treatments, and X-rays, if available. Please consider contacting your previous dental office and authorizing the transfer of your records to ensure seamless continuity of care. We have a convenient Release of Records form available, which you can send to your previous dental office if they require an official written request.

Financial and Billing Information:  Our office operates on a fee-for-service policy and full payment is due at the time of service, unless prior written arrangements have been made with our office. Upon your arrival, please ensure that you have your preferred method of payment ready. We accept cash, as well as most major debit and credit cards.

Insurance Information:  To ensure clarity, we would like to inform you that our office follows a non-assignment of dental benefits policy. If you have dental insurance, we kindly request that you bring your insurance card, policy details, and any pertinent information related to your dental plan. It may be helpful for you to contact your insurance provider prior to your visit to confirm your coverage and familiarize yourself with any co-payments, deductibles, and any other plan limitations that may apply. As a courtesy to our patients, we are happy to assist you by filing dental insurance claims on your behalf.

Arrive at least 15-30 Minutes Before Your First Appointment

When you visit us for the first time, our team will take comprehensive records of your personal and health history so that we have a clear picture of your oral healthcare and your needs. We kindly ask you to set aside a generous amount of time, approximately 1-1.5 hours, for your first visit to our office.

It’s important to note that initial appointments typically require more time as they involve several important steps. These steps include patient registration, reviewing your health history, conducting and analyzing radiography, performing a clinical examination, creating a treatment plan, and engaging in informative discussions. By allowing for enough time, we can ensure that none of these crucial procedures are rushed.

We want to inform you that 15 minutes of your reserved appointment time will be dedicated to completing the new patient paperwork. However, to save time or if you anticipate needing extra time, we recommend filling out these forms in advance, before your scheduled appointment. Moreover, we suggest arriving early and coming prepared with all the necessary information, so you have ample time to complete the check-in process.

Arriving at least 15-30 minutes early for your dental appointment helps us to address any issues with your dental insurance and/or medical history prior to your appointment time with our dentist. Even if you have already completed the paperwork and have no questions, it’s still a good idea to show up a little early to your appointment so you do not feel rushed and it will ease any dental anxiety you may have.

Arrive Prepared

To make your visit as comfortable and convenient as possible, we kindly ask you to take care of a few things beforehand. Please ensure that you have eaten, brushed your teeth, and used the restroom before arriving. We want you to feel at ease during your time with us.

Patient Information & Our Office Policies

Part of providing safe, personalized dental care involves getting to know you and reviewing your medical history. We ask all patients to complete mandatory health forms prior to their first appointment so that we can prepare your experience and offer expertise tailored to your needs.

Beachstone dental is a fee for service office and our fees for dental services are generally based on the current year’s fee guide as set out by the Ontario Dental Association. Full payment for dental services is due at the time of service unless prior written arrangements have been made with our office. Please bring your payment information with you to your dental appointment. We accept cash and most major debit and credit cards.

All professional services are charged directly to the patient and our office has a non-assignment of dental benefits policy, as recommended by the Ontario Dental Association. If you have dental insurance, a dental plan can be a great help in covering the cost of dental care. Beachstone Dental accepts most major dental insurances and our office will be happy to file most dental claims online on your behalf as a courtesy to you.

Guidelines for Minors and Patients Under Guardianship

For the best experience and your loved one’s well-being, we’d like to request that if you’re bringing a minor or an adult under guardianship to their appointment, please have a responsible adult, like a parent or legal guardian, accompany them. This way, we can ensure top-notch care and your peace of mind.

For a seamless experience, we might ask parents or legal guardians who come along with the patient to fill out a form and show a government-issued photo ID. It’s just a simple way to make sure we know who you are and your connection with the patient.

To ensure the safety and comfort of our young patients and their families, we kindly ask that only one adult, such as a parent or legal guardian, accompanies the child to their dental appointment. If your child has siblings, we request that they do not accompany the visit unless absolutely necessary. This helps us focus on your child’s care and ensures everyone’s well-being. Due to the ongoing novel Coronavirus (COVID-19) pandemic, compulsory precautions and additional safety protocols are currently in place to provide a safe and seamless dental appointment to all our patients.

When scheduling your appointment, you will be pre-screened for COVID-19 and screened again on the day of your appointment.

Prior to your appointment – if you test positive, develop symptoms, and/or have been exposed to COVID-19 and asked to self-isolate, please call our office as soon as possible so we can proceed accordingly.
